The Role:
Facilitating all NDIS admin work, including case managing and support coordination. Answer enquiry emails from support coordinator/case manager/participant.
Prepare service agreements for participants.
Create the service bookings on the NDIS portal if participant is NDIA (Agency –Managed).

We are seeking a support coordinator for our Burwood/ Castle Hill (part-time) location.

Responsibilities include:

Check for any NDIS/Disability and Aged Care potential referral, received from our provider.

Send this out to assist with the application. Call to follow up the referral.

Audit and maintain the spreadsheet for any inactive cases. Liaise with provider/participant to

re-book or find out any issues about the case.

Follow up the Comprehensive Report with the provider for plan review - call the support

coordinator and participant to re-engage the service after plan review. Work out a revised

budget for the new NDIS plan.

Bilingual (Korean ) ideal, English is a must.

Support Worker
Key Responsibilities and Duties:
Daily Living Support: Provide 1:1 physical assistance and consistent verbal prompting for personal care tasks, including showering, dressing, and grooming.
Mobility Assistance: Offer standby assistance to physical assistance with mobility and transfers. Encourage the consistent use of prescribed mobility aids (e.g., four-wheel walker, wheelchair) and assist with safe navigation on uneven surfaces or steps. The participant occasionally chooses not to use her four-wheel walker and instead holds onto Support Workers' hands for assistance while walking.
Cognitive & Executive Functioning Support: Provide consistent 1:1 support for attention, memory, planning, problem-solving, decision-making, and safety awareness due to significant cognitive impairments. This is crucial for tasks like grocery shopping, meal preparation, laundry, domestic cleaning, and financial management.
Community Participation: Provide 1:1 support for safe access and engagement in community, social, recreational, and economic activities, addressing challenges like impulsive behaviours in public settings.
Night-time Support: Provide inactive overnight support to assist with re-settling when the participant wakes or exhibits behavioural challenges.
Essential to have driver's licence and own vehicle.
